You only need to complete a Calculation Order if your QILDRO contains percentages. A Calculation Order can be completed by you, your alternate payee, or an attorney.
The Calculation Order translates the percentages in your QILDRO into final dollar amounts. IMRF cannot calculate these amounts for you. We can only pay your alternate payee once we have a valid court-certified Calculation Order that tells IMRF the exact amount(s) to be paid.
To properly complete your Calculation Order, you need the following:
- The court’s judicial district and county.
- The case caption and case number.
- The name, mailing address, and Social Security number for you and your alternate payee. (Note: We will accept a Notice of Confidential Information Within Court Filing in place of the Social Security Numbers (SSNs) on your QILDRO. Discuss this option with your attorney if you do not want SSNs to be included in the court record. You will need to include SSNs on the Notice of Confidential Information Within Court Filing; however, this document is for IMRF records and does not get filed in court.)
- Your relationship to the alternate payee.
- A copy of your QILDRO.
- The Illinois Circuit Court judge’s signature.
- The Illinois Circuit Court Clerk’s certification. This must bear the clerk of court’s seal or stamp certifying the document as a true and complete copy of the original.
- A check or money order payable to IMRF for $50.00 for the required processing fee.
You will need to refer to your QILDRO when completing the Calculation Order. The Calculation Order has separate sections for each benefit type and calculation method you included in your percentage QILDRO. You must complete each section in the Calculation Order that corresponds with a completed section in your QILDRO:

Make sure your Calculation Order is valid
To make sure your Calculation Order is accepted by IMRF:
- Use the form that IMRF provides. Do not retype the form.
- Submit the entire form, including sections that you left blank.
- Fill out the appropriate sections that match your QILDRO.
- Do not add, change, or cross out words, or customize the form in any way.
- The form must be signed by a judge in an Illinois Circuit Court.
- The form must be certified by the Clerk of an Illinois Circuit Court.
- You must include the required $50.00 processing fee (a check or money order made payable to IMRF).
Have IMRF review your Calculation Order before you go to court
If you would like IMRF to review your Calculation Order before you take it to court, you can mail or fax a copy to IMRF’s legal department at 630-706-4656. We will review your form to make sure it has been completed correctly.
Please keep in mind that we will not:
- Check your math.
- Verify that what you have entered is how you intended your benefits to be divided.
If you have questions about completing your Calculation Order, please call IMRF before you take your Calculation Order to court. Keep in mind that IMRF can answer factual questions about your benefits and Calculation Orders, but we cannot advise you on how to divide your benefits or do any calculations for you.
Submitting a Calculation Order
Once you are sure your Calculation Order has been completed and certified properly, you should submit it to IMRF along with the required $50.00 processing fee. Send the Calculation Order to:

Make sure to keep a copy of your Calculation Order for your records.
After we receive your Calculation Order, we will either:
- Send you a letter that it has been accepted.
- Send you a notice that it has not been accepted. This notice will include what corrections you need to make before we can accept it.
You must make all required corrections to your Calculation Order before IMRF can accept it.
