Enclosed is one copy of IMRF’s 2004 Disability Benefits booklet. Revisions to the booklet include:
- Updated Frequently Asked Questions (page i)
- Revised explanation of disability benefits for a condition with a pre-determined time period (page 14)
- New chart explaining when a member should apply for Social Security disability benefits or a Social Security Old Age pension (page 24)
We are sending one copy of the IMRF Disability Benefits booklet to each employer. Please retain this booklet as your file copy. However, if your employer has multiple locations, you can request one copy of the Disability Benefits booklet for each location.
You do not need to maintain a supply of the Disability Benefits booklets because IMRF mails the booklet to a member after we receive any one of the following disability forms:
- Form 5.40, Member’s Application for Disability Benefits
- Form 5.41, Employer Statement - Disability Claim
- Form 5.42, Physician Statement - Disability Claim
In addition, IMRF will mail a Disability Benefits booklet to a member if he or she calls and requests one.
