IMRF Termination Form 6.41

Form 6.41, "Termination of IMRF Participation," must be filed for members who change from an IMRF covered position to a non-member (Social Security only) position, or for all members who terminate employment. This form should be completed online through your Employer Access account.

Termination includes resignation, dismissal, retirement, death, term expiration of elected officials, and extended military leave.
