IMRF Termination Form 6.41

Form 6.41, "Termination of IMRF Participation," must be filed for members who change from an IMRF covered position to a non-member (Social Security only) position, or for all members who terminate employment.

Termination includes resignation, dismissal, retirement, death, term expiration of elected officials, and extended military leave.
